• How do I get to your office?

We are at 3700 N Kickapoo Suite 124. We are North of MacArthur.  North of Dollar General and on the West Side of the Street.  A Picture of of the building is at the top of our site.  We have a "FMS" logo on the front of our building. 

• Is an appointment required to be seen by a provider?

 During the week an appointment is required in order to be seen by a provider.  If you need to see a provider on the weekend, no appointment is needed.  See the bottom of the page on our home page for details.    

• Do I need an appointment for lab work during the week?

You do not need an appointment during the week for lab as long as you have an order from your ordering provider for that lab. 

• Can I have lab done during the weekend?

There are a few lab test that the nurses can perform during the weekend.  But most of our lab is not available during the weekend unless it is urgent then the patient would be refered to the hospital for urgent services.

• What do I do if I need a refill of my medicine?

Call your pharmacy where you filled it at and request a refill.  If it is out of refills, ask the pharmacy to fax your a request to your provider.  We ask that you contact your pharmacy at least 24 hours prior to running out of medication to allow time for communication between the pharmacy and your provider.
