Our web site now has the ability for our patients to make account payments on line.  Simply click "Pay my Bill Online" at the top of the home page to the right of the FMS logo and register to make your payment on account. 

You can now either print out your new patient forms or fill them out on line and submit them to our office prior to coming in for your first appointment, for your convenience.  Simply click on the "Health Forms" link to the right of the FMS logo on the home page and follow the instructions. Be sure to fill out all 4 forms prior to coming in. They will all need to be completed in order for registration to be completed.
