We will begin giving flu shots on Wednesday October 15, 2008 to our patients 4 years old and up.  We have not received our flu shots for patients under 4 years of age or the vaccine stock for our SoonerCare Patients.  We will keep you posted as soon as those are received.  No appointment is needed.  Flu Shots are to be paid at the time of service and the charge this year is $25.00.

Our web site now has the ability for our patients to make account payments on line.  Simply click "Pay my Bill Online" at the top of the home page to the right of the FMS logo and register to make your payment. 

You can now either print out your new patient forms or fill them out on line and submit them to our office prior to coming in for your first appointment, for your convenience.  Simply click on the "Health Forms" link to the right of the FMS logo on the home page and follow the instructions. Be sure to fill out all 4 forms prior to coming in. They will all need to be completed in order for registration to be complete.
